Here are the 3 MOST dangerous misconceptions home health nurses hear every day...
“There is no template or actual format for writing OASIS nursing narrative notes.”
Actually, that sounds so “WHACK”?! You and I both know that there MUST be an OASIS nursing narrative note “gold standard” that Medicare and CMS are using to measure your submissions. Whether others in the home health industry want to call that “gold standard” a “template or format” is completely up to them. What I know is that Medicare and CMS have laid out very specific requirements for ALL of their expectations. Do they make those requirements easy to find or understand...not at all, (but that’s for another discussion). The point is, that “gold standard” is your template (whether you can find it or not...it exists) and while it may not give you an exact flow or specific formatting details as they’ve done for the requirements...they do expect you to use your common sense and grammatical education to format their specific requirements in a way that makes sense to THEM, that makes it EASY for THEM to see that you’ve included everything and that that everything meets their home health service reimbursement qualifications.
“All OASIS documentation must be objective and/or subjectively individualized to each of your patients, so you can’t use a template to write them.”
Okay, there is a difference between using a template to write your OASIS nursing narrative notes and writing the same exact thing for every patient that has the same ICD codes or nursing diagnoses. For example, a template just gives you the requirements for what needs to be included and where to place it. Using a template does not mean that all of your patients that have ICD codes or nursing diagnoses of Atherosclerosis and high blood pressure will then be educated on or recommended for Warfarin and weekly PT/INR. Each patients assessment level and interventions depend on their INDIVIDUAL health history, co-morbidities, past, and current medication regimen, etc., and accounting for each of these in addition to their ICD codes, nursing diagnoses, and observations (objective data) along with their statements (subjective data) during our OASIS data analysis and 485 planning makes each and every patient’s OASIS nursing narrative note unique and individualized.
Yes, your agency may have a specific process they’d like you to follow when dealing with patient documentation matters. However, Medicare and CMS set the “gold standard” for your OASIS nursing narrative note as well your OASIS tool submissions in its entirety. Therefore, the OASIS requirements DO NOT CHANGE based on your agency, but the amount of reading or reviewing, or paperwork that your agency reviewers are willing to digest may not always be on par with what Medicare expects. Meaning, just because you get a remark from Debbie at your agency saying “Your nursing narrative notes are always so long, with details you really don’t need.” ( I know this remark ALL too well because it’s been said to me...side-eye) That doesn’t mean that it's wrong or that you should shorten it..it means, Debbie doesn’t like reading long nursing narrative notes, which isn’t your problem because your documentation is done to the expectations of Medicare and CMS, in their language, to cover your nursing license as well as to provide the best health care outcomes for your patients. In short, your training should be with someone that has experience with and understands OASIS requirements as dictated by Medicare and CMS from the point of view of a home health nurse.

DOES THIS TEACH ME HOW TO WRITE OASIS NURSING NARRATIVE notes FOR ANY TYPE OF PATIENT DURING ANY TYPE OF OASIS ASSESSMENT?
I'm going to get you as close as possible to the "perfect nursing narrative note". After years of working on OASIS nursing narrative notes for myself then moving into the agency and individual private student spaces and working on dozens of student narrative notes, one of the biggest problems I saw was the lack of actual understanding of OASIS and its significance to Medicare and your patients home health care needs. That's why I made NNRB. So if you (correctly) consider that understanding OASIS is part of writing your nursing narrative, then YES. I'm not going to leave you hanging—after you get through the "understanding" sections of the blueprint, I've hooked you up with the formula to arrange what you've learned as well as a number of sample narrative notes for you to use as guides.
HOW LONG WILL THIS TAKE ME TO DO?
This is my read-through-in an hour blueprint.. Will you write 50 OASIS nursing narrative notes in a day? NOPE! Because I don't believe in that, and if someone's telling you that makes for career advancement, I want to see their nursing license track record and list of past agency employers...point, blank, and the period! :)
